If you are a new patient please visit our Patient Forms page to download our new patient bundle. Printing the forms, filling them out, and bringing them with you will allow us to attend to your dental needs in a more timely manner then completing them upon arrival. Payment Options
For your convenience, we accept cash, personal checks, money orders, most major credit cards, and care credit. Payment is expected at the time services are performed unless previous arrangements have been made. Feel free to contact our office if you want to discuss financial arrangements.
